By completing this form, I hereby grant ThreePoint Custom Designs LLC a non-exclusive, irrevocable, worldwide, royalty-free, fully paid up license in perpetuity to use, reproduce, and publish the name, likeness, image, appearance, or other indica of public personality (collectively my “Right of Publicity”) on apparel ordered through this order form, and for promotional purposes, including use on social, digital, and print media platforms (e.g., a website, Facebook, Instagram, Twitter, and other marketing and advertising materials). ThreePoint warrants that the licensed Right of Publicity will be used solely for the item(s) ordered and not resold to the general public. I understand and specifically agree that ThreePoint will have exclusive copyright ownership of any materials produced under this agreement and that I will not receive any compensation, payment, or royalty for use of the licensed Right of Publicity. I hereby waive and release ThreePoint from any claims, known now or arising in the future, related to its use of the licensed Right of Publicity. If the individual is below the age of majority, I confirm that I am the parent or legal guardian and agree to these terms on their behalf.
